Notification of students leaving

If a student is leaving Highland Reserve State School during or at the end of Term 4 (excluding graduating Year 7 students) please
contact the office and complete a Student leaving form (PDF, 156 KB) before their final day of school.
Notification of students who will be absent at the commencement of school in 2013.

A notification will be required for students who are returning in 2013 and will be absent from school when it resumes on Tuesday 29th January 2013.
Please notify our office before their last day at school in 2012 on (07) 5588 3333 or via email at office@highlandreservess.eq.edu.au.
Please provide name of student/s, dates absent and absence reason.

Students with medication at school

If your child has any medication at school including Asthma puffers, epi pens, medicines or tablets these must be collected by a parent from the office on your child’s last day of school for 2012.
If the parent is unable to collect it themselves then a letter in writing is to accompany another nominated adult to collect any items stored in the healthroom.

Students will not be allowed to collect medications of any sort.
Any medications still in the healthroom after the students last day of school in 2012 will be disposed of and not held on the premises over the holiday period due to security reasons.

Please ensure that if your child needs to continue, or start medication at school in 2013, a new Request to administer medication form (PDF, 128 KB) and Medical details form (PDF, 76 KB) is to be completed upon return next year.
Also ensure that the medication being sent to school is not about to expire and has a pharmacy label and action plan attached to it.
